I was playing Deathslinger and i realized when i downed the obsession that i still had 5 stacks, so the next time i found her i tried to break the chain, and the stacks remained the same, so it's not working as "every time a survivor loses a health state", it's working as that "except if the survivor uses plot twist or the Deathslinger breaks the chain".
