How did this Nea enter the dying state?
This is one of the most baffling things I have ever seen.
This Nea randomly went down. She screamed and triggered the generators I had marked with Eruption to explode.
I do not remember hitting her off hook to make her mend. I generally try to avoid camping and tunneling and in circumstances where I do return to hook, I aim for the unhooker.
Nothing in the match made me suspicious of cheaters which could have forced her into the dying state, everyone seemed to be playing normally.
The map was Coal Tower 2, though I don't think the map matters, since I don't think there's any environmental effects which can injure survivors.

Maybe a cheater who tried to burn your anti 3-gen cap as quick as possible so you cannot kick gen anymore and not be able to used 3 perks of yours

I don't think anyone in the lobby was hacking, since I was able to kill them all (I let the Jeff go). I also wasn't holding a 3gen at this point, this happened at around 3ish generators left to go.

Maybe you did hit her after being unhooked but didn't notice? She could have body blocked and you thought you hit someone else and then she forgot to mend? I've seen that a few times.

The entity punished her for running Self Care...
Yeah, you have 10 seconds of Endurance off of the hook by default. If you get hit in this time, you don't go down and you get deep wound instead.
