PC (Steam) - Some Pinhead Chain Portals Don't Disappear

Like the title states, I have been experiencing while playing Pinhead that some of the portals that open when chains are thrown don't disappear. The picture below is an example of one of the many portals I've come across.

Step 1.) Boot the Game
Step 2.) Play as Pinhead in a Public Match
Step 3.) Play the game for a while, allow the chain hunt to happen and use Pinhead's power
Step 4.) Witness random portals that don't disappear like the picture above.

This doesn't appear to be addon-dependent but MIGHT be perk-dependent. Perks used are Nowhere to Hide, Coup de Grace, Trail of Torment, and A Nurse's Calling.

    it is not pwrk dependant at all. I used 4 different perks than the one listed here and it still happaned to me . Just another pinhead bug is all.
