PC - Broken hook on Red Forest

In a three-man SWF, some friends and I were playing a normal match against the Knight in Red Forest - Temple of Purgation. We all played on Steam and are currently up to date on versions.
In the photos you see, our friend, who played as Ada, was hooked. However, both my friend, the Nicolas Cage, and I, the Zarina, physically couldn't unhook her. It wouldn't give us the option. We don't know what caused it but here's the step by step if someone wants to try to recreate it.
Step 1: Spawn into Red Forest - Temple of Purgation.
Step 2: Get downed and hooked
Step 3: Get hooked on a specific hook near the Temple
Step 4: Die because you cannot be unhooked
