Forgotten Ruins Game Breaking Bug

Our teammate multiple times has gotten stuck in one of the portals in Forgotten Ruins. They teleport, and get stuck in the output side of the portal. They are unable to do anything, they can sometimes be downed if the angle is just right, but cant be picked up and cant escape the portal.
This has happened 3 times this week. We have noticed that whenever it happens, there is a generator or hook on the opposite side of the wall as the door.
Killers and survivors can enter/exit the portal just fine from the other side, but the stuck survivor stays stuck.
This happened to a survivor on PS5.
