Dead Hard not giving endurance while inflicted with "Deep Wound"

While previously deep wounded by any means, the perk "Dead Hard" does not give the endurance effect.

    It still lets you use your token for Dead Hard and exhausts you, but does not give endurance.

    Endurance does not activate while in deep wound to prevent endurance stacking. When in deep wound any hit will put you into the dying state no matter what perks you have.

    There was also a bug in the latest patch where endurance perks were not working correctly. Basically, OTR and DH were just not giving endurance iirc.

    I'm not sure if this is the problem or if that issue was fixed.

    Yes that's how it works and always have.
