On PC.
Pretty simple, just wait long enough (2-3 minutes) on the end-game scoreboard after playing a match on Shattered Square and slowly that same Shattered Square you were just on will come into view on the top-left screen (I only know this because I decided to give props to the Survivors I went against cause they didn't deserve to all be Mori'd). After its grand appearance, it'll crawl its way down diagonally to the middle of your monitor.
Below image for context. On the top left above the scoreboard, you can see tan splotches which I believe is the map in question. Sorry that the image is in such poor quality, as I play with low specs.
