Getting stacks with Sole Survivor Perk with no one dead

Hello i played a game recently with the perks : lithe, empathy, wake up and sole survivor
PLATFORM:
On PC, using Steam Version on patch 8.2.2
Killer info : I was against a Wesker
None of my allies died but when they escaped the last one escaped by crawling and when he finished to escape i got a stack on my Sole Survivor perk.
usually this perk get a stack when someone is dead (by mori or hook) and having only 1 stack is weird, i should have 0 or 3 because my 3 allies were in the same statement, so it's maybe because one escaped by crawling idk
none got the hacth, i was the last one and escaped by the door
