I am getting an extra hook state when being unhooked from my first ever hook

SUMMARY OF THE ISSUE: Only being able to be hooked twice as you get an extra hook state sometimes after being unhooked even though its your first hook, additionally it happened to my friend in a separate game.
PLATFORM: PC
STEP BY STEP INSTRUCTIONS ON HOW THE ISSUE OCCURED: I got hooked by the killer, waited on the hook for about 20 seconds and i got unhooked, it appeared on my screen as me having two hook states instead of just one as it was my first every hook.

Did you or your friend use the perk Shoulder The Burden at any point in the match, and accidentally took a hook state from another survivor and transferred it to yourself?
