SUMMARY OF THE ISSUE:
I think what happened was that Jake was unhooking Sheva getting her somehow stuck in the unhook animation and same time Jake just vanished from the match. Like it showed him alive but could not be spectated and game did spawn the hatch when she was the last alive stuck on the hook even if it also showed Jake still not killed or sacrificed. You could also not unhook Sheva anyway did not give you the option.
PLATFORM: PC
STEP BY STEP INSTRUCTIONS ON HOW THE ISSUE OCCURED: I am not 100% sure how it happened. My best guess is that Jake either did something shady or lost connection when unhooking Sheva from the hook causing her to get stuck. She then dc'd too because was stuck.
