Exultation has inconsistent activation

SUMMARY OF THE ISSUE: Exultation will not activate when holding an item consistently. Any time a killer is mid swing or holding a survivor, if they are successfully hit Exultation will not upgrade my survivor item. The only time it works seems to be when a killer his stunned and not in the middle of any other animation.
STEP BY STEP INSTRUCTIONS ON HOW THE ISSUE OCCURED: Slap killer in the middle of various pick up and attack animations and it does not activate.
Hit them while they are walking through a pallet and not doing any other animation works 100% of the time. I'm aware of the cooldown timer. This isn't that.
