Survivor not dying on hook when they should

SUMMARY OF THE ISSUE: I had the last 2 survivors on hook at the same time and but 1 of them didn't immediately die when they'd both progressed to the 2nd stage. Geralt abandoned as I was picking him up, so I don't know if that was the cause or not.
