STEP BY STEP INSTRUCTIONS ON HOW THE ISSUE OCCURED:
On the offering screen, during the final animation where the offerings disappear, you can trigger the mouse over tooltips to see what hidden offerings are.
Screenshot below is me playing survivor, the killer bought a mori offering.

Update: You don't need to wait for the final animation - finding the right spot with your mouse can reveal the offering is
