I actually jokingly once recommended an achievement for killing all 4 Survivors with Rancor after DS got changed. Still possible, but extreme difficulty.

Schrodinger's Phantasm: Down X Survivors within 8 meters of an active Phantasm that you can teleport to as the Hag.
Deadly Statue: Down X Survivors without reaching full movement speed as the Spirit.
No Escape: Catch X Survivors in a trap within X seconds of them escaping another trap as the Trapper.

I think it’s safe to say that while fun to think about none of these so far would make it in. I was thinking about doing some of these for fun, and I don’t want to so far.

Hunt from the dark - Successfully continue chasing and hit a survivor while blinded 50 times
Never Give Up - Escape the trial in the dying state after being downed outside the exit gates, then becoming the last survivor.
Behind you... - Successfully hit a survivor into the dying state when they're not looking at you or in a chase. Grabs count.
Breakdown - Have 5 generators regressing at once with all 4 survivors still alive.
Decimation - Complete a trial, killing all survivors, in under 5 minutes
I'm outta here! - Escape the trial in under 5 minutes
I'll be here all day - Escape into the hatch while downed as it opens.

Whiffing To Success is similar to that. You have to make the killer miss and vault (like when you vault and the killer swings and misses).

Solo Exit:
Close Hatch to prevent a 4 man hatch escape before the other three survivors can you jump into the hatch then sacrifice the remaining survivors.
Exceptional performance
Without any perks or add-ons sacrifice all the four survivors before a single gen is completed. ten times.

Fraidy Cat - Don't attempt to repair a generator for 3 minutes while injured and not in a chase
Potato - Escape and have both Lightbringer and Benevolent emblems of bronze or silver
Unappreciated - 3 or more generators are repaired while you are chased and no one unhooks you
All About Me - Escape through the hatch before the EGC begins leaving behind at least one survivor
Karma - Get killed after teabagging in the exit area

@AshleyWB Yeah, they're like mini challenges which give tidbits like lore and cosmetics, and works like the bloodweb (except to take a node you need to do it's respective challenge.)
This also just gave me a question about the lore, and a blue bloodweb... possible 2nd entity/outside source confirmed?
