I have had this happen to me 3 different times. from what I can tell, it has to do with someone being very laggy and joining the lobby after the lobby things they timed out, but joining at the same time as someone replaces them while there is no killer.
Be sure and unequip your offerings when this happens, as they still burn... though you do get an automatic 5k bp for survival, so maybe burning a low survival offering like a 50% would be beneficial.

It appears to have put me as the literal 5th wheel, but i am the bill in the image.
there was no killer and my name was not on the list of people.
not sure if can be reproduced
first time happening.

It completely messed my game up after said connection to the party was lost i solo qued and i had to restart the game. This game is in dire need of attention behavior seriously should have a team dedicated strictly to console issues would be so much better.

Oh I can't comment on console. This happened to me 3 times on PC.
