Do survivors loose points for letting a survivor get to struggle state?
Do they? For example I am fixing a gen and a survivor gets hooked and they get to second state because nobody went to help them out, do they loose points? If they don’t they should. I have had tons of games where nobody would come and rescue me because they were all on gens or someone is getting chased and the other two are doing gens. This has happened when I am getting chased and nobody goes for the person on the hook and they end up letting go, which I totally understand.

Only via opportunity cost (as in, you lose the opportunity to gain more BP).

If you mean emblem points, everybody loses Benevolent emblem points when somebody gets hooked and everybody gets those emblem points back when they get unhooked, regardless of how long it took to do it. If they die, you don't get the points back unless they died within 10 seconds of being hooked (so it was their last hook or they let themselves die on the hook shortly after being hooked)
To answer your question: no, not really. The survivors lose/gain the same amount of points whether they rescue the survivor in the first or second phase. The only difference being that since the rescuer gets bonus emblem points, letting the survivor go through a phase without getting saved means that's one less instance where someone could get those bonus points.

So now you want to subtract BP from the other survivors when a killer is camping the hook? No thanks!

Simple fix is to make it so they dont lose points if the killer is near the hook.

I don’t see how that’s enforced. How long does the killer need to be at the hook for the penalty to not take place? What if the killer is proxy/patrolling....as opposed to outright facecamping? Do those survivors lose points? If they just go for it for so they don’t lose anything, and the killer comes back and downs the unhooks guy,.. will they get criticized for farming even though the game forced them to go for the rescue? I think it would be way too complicated, you can’t force survivors to go for a save...especially when killers are aware those survivors will lose bp. That will just entice them to stay around the hook area.
