Survivors can become infected, take a physical hit, then purge their injury away to full health.

Also, sometimes the purge attack indicates it hits survivors, but yields no results.

The first one is easily done. Get hit, get puked on, go heal at a fountain.

The second is not so easy. Replicating this is hard, but it happens mostly when the survivors are running.

Survivors could always use the fountains to heal but only if they are infected. This works if they are injured or broken from infection.

The vile purge/ corrupt hitting survivors and not doing anything is from latency. I found when I turned off cross play this problem became less frequent.

First one is not a bug, this is intended by design.

Second one, could be connected to latency - possibly.

I should of elaborated. You can hit a survivor with your weapon, and they can heal themselves to full health using a fountain. I don't think taking a bath heals getting smacked with a blunt object but that's just me.

That is also intended, the fountains always heal the survivor regardless of where the damage came from.
