Breakdown buff
This is an idea that I thought of to buff this perk.
In addition to its current effects, when you are hooked on a Scourge Hook the relevant perks do not activate.
With the new perk Pain Resonance coming in, I think Scourge Hook perks will be used more often in the future. So I think it would be a nice buff to make Breakdown counter these perks, since Breakdown is not a good perk nowadays.

The scourge perks are already so situational, I'm not sure.

Well I predict Pain Resonance will be a popular perk when it's released, plus they'll make more Scourge Hook perks in the future.
However, the other suggestion about it working on rescues is a good idea.

Yes, you need to introduce breakage mechanics into the hook itself for at least 60 seconds
There are times when someone saves and falls to the ground in the same place and the killer just hangs him on the same hook
