Should Sloppy Butcher trigger on Special attacks?
Like they did with Hex: Third Seal?

Given that all healing slows have lost most of their value due to CoH...
I don't see why not as it doesn't really matter at this point.

Honestly, most of the "do something else when hit" perks should trigger on special attacks too. It's not like they tend to have amazingly strong effects.

I would use it for every perk like Surge, Frenklyn's etc. except Exposed status perks.

I think sloppy is actually okay as being basic attacks only. There's some other basic attack perks I would change to work with special attacks though (surge and remember me come to mind).
While it's true that sloppy has lost a lot of its value nowadays with the healing meta, I think that's more of a problem with COH being overpowered, not sloppy butcher being too weak.

One problem could be how Franklin's currently works, the item is dropped at the feet of the killer and not the survivor. Imagine getting hit by a ranged killer and then having your item teleport to the killers position.

I support this suggestion. Would make certain Killers better, however we need to address a few Killers for their interaction. Still good idea.

There's a lot of killer perks that should work with special attacks, like:
- Dark Devotion
- Gearhead
- Surge
- Knockout
- Sloppy Butcher

I would be more than happy with your list happening. Agree with what some others say about Franklins...no need for that perk to be buffed.

I never really got why most of these “basic attack only” perks didn’t work for special attacks. I’m sure the devs have a reason, I just don’t know what it is exactly. I can see why maybe the Exposed condition should only work for basic attacks, but is it actually overpowered if Sloppy Butcher lets Victor or Huntress or even Trickster inflict Mangled or Blindness?
Speaking Trickster, I could see an argument that these perks should only apply their effect when they inflict a damage level. So a single Trickster dagger probably shouldn’t inflict Mangled or Blindness, but the dagger hits which pass the threshold to actually injure or down someone could.

They'd have to do something to avoid invalidating any and all add-ons that inflict Mangled.

Also, it most certainly does not if you propose stacking them. Not to mention that if you're only using that version of Sloppy, then all the add-ons that inflict Mangled are just pointless wastes of bloodpoints.

It would take into effect twice if you hit with a special attack

Unfortunately, that lazy design is common on most of the reworked addons. We have hatchet, bear trap and Bubba chainsaw that inflict Mangled. We have Shop Lubrificant (Bubba) that his a low level Knock Out. We have Metal Spoon (Nurse) that's basically Stridor. Black Box (Nightrmare) that is too a low level Blood Warden.
Personally, most of them should trigger on special ability. I don't see the point of having a "special attack" as killer and don't create fun build around it cause the perk don't trigger for it. They did it for Third Seal (i used it with Legion, fun times), don't see why they can't do it for the other perk.

That does not mean it makes sense to have a perk and an add-on that do the exact same thing. Again, lazy design.
At least the ones that inflict Mangled have some sorta purpose to exist since Sloppy currently only works with basic attacks (which is why I'm not a fan of OP's idea). But yes, I get what you mean.

Yeah. For Special Attack is basically all attack that inflict damage (Hatchet, Legion FF) and maybe yes, they should apply Franklin. But not if the item is dropped on killer feet. I don't use it a lot, so i didn't even know.
Yes, but if they change, in this case, Sloppy to activate even on special attack, you can decide to actually use sloppy as a perk, and take other addons, or sacrifice one addons for only the mangled effect (the addons don't give hemorrage) and equip a different perk.
