Its not a bug! I don't have a link or screenshot ready but if you check the last update's patch notes there's a whole update on how items work.
I don't remember exactly how it's explained but basically you can carry temporary items (flashbangs, EMPs and other killer items, white glyphs) as the same time as a regular items (medkits, flashlights...)
Honestly I think it's a nice buff, it was quite annoying that using a perk basically stopped you from bringing an item and I don't think it's going to make flashbangs OP. The whole item system changes are nice, I'm particularly happy because it was very tedious to have to put down your item, pick up and use a killer item and then pick your item back up again.

The only thing I don't like is that it hides the flashbang, yet you can throw it down. I'm fine with holding two items, but just not exactly with a hidden flash that someone can just run up and flash you with.

I didn't pay attention to that, I understand that it has to be hidden if you're carrying an other item but is it still hidden when you don't have one ? If so I agree a survivors without an item should definitely be visibly holding the flashbang, it'd be weird it they didn't.

Nope, it's a temporary item because survivors don't keep them even if they escape with it.
And I'm fairly certain it's intended for survivors to carry both an item and a flashbang at the same time considering not having to drop your item to make one was in the patch notes
