Ultimate Weapon is Bananas
What were they thinking when they designed an info perk that gets around distortion AND works on-demand? Why would you ever equip BBQ or Whispers over this? Seeing all survivors at the start with lethal pursuer will net you exactly one chase every game, Barbecue can potentially get no value if the survivors are hiding or doing gens in safe zones, Whispers only gives you a vague idea that there are one or more nearby survivors, and spies is pretty much completely random in terms of value. Just gut this no-effort value engine perk for the sake of the game, please.

It wasn't until I saw it paired with Dead Man's Switch that I realized how powerful it is. Making them scream and forcibly removing people from generators on demand is pretty strong for delay.

Couple reasons why you'd pick other info perks, at least in my opinion.
For starters, auras are a much stronger form of information that screams. For BBQ specifically, it's also much more passive than Ultimate Weapon, since it doesn't require you to go out of your way at all.
There's also a lot of perks that give you less information, but far more relevant information. Discordance, for example, isn't on-demand but gives information that you're always going to want to know about when it happens. Gearhead gives information on which gens are being worked on, and has the added benefit of chewing through Distortion stacks pretty aggressively.
The biggest reason, at least for me, is that most other info perks don't also tell the survivors you know where they are. Ultimate Weapon is one of the most over-hyped perks in the game; it gives the weakest form of information (tied with loud noise notifications), it tells the survivors you have it, it has anti-synergy with stealth, it requires going out of your way to go search lockers, and it has an annoying dual-cooldown system. Individually none of those things are perk-killers, but together? I'd much rather use something else.

Whispers has been a bad perk for years now. BBQ, like most aura perks, works without the survivor having any knowledge of it working, which gives the killer an element of surprise, and that's one of its advantages over Ultimate Weapon. Ultimate Weapon seems more powerful than it is because survivors know that the killer knows where they are. Perks like BBQ give the survivor's position away without them knowing. That's why it's powerful on characters like Ghostface and Huntress. It allows the killer to catch the survivors relatively flat-footed. Ultimate Weapon never does this, because survivors always know when it's been activated. It's like a single sonar ping with the killer at the center. You get a read on all survivors' positions now and only now. Aura perks like BBQ are x-ray vision, you see survivors without them seeing you, and you get a sense of where they're going.
Ultimate Weapon does also have a hard counter in the form of Calm Spirit, which, unlike Distortion, always works in exchange for the penalty to certain action speeds.
Edit: Forgot to add about Spies-it's kind of a training perk to get newbies to understand that the birds can help them. At least, that's how I see it.

It's disgusting on Killers like Nurse, Blight or Oni who can down survivors really fast and snowball one by one.

In a game where almost no strategy for killers that simply exists, it only makes sense why this perk is around.

There is actually not that many killers, where I use Ultimate Weapon over other info perks.
I use it on Oni (kinda broken for him) and Deathslinger (can't waste any time looking for survivors).
But for most killers aura reading is simply better. Ultimate weapon is broken on fast killers, which could be fixed by simply decreasing duration after first scream, so you can't go around whole map. Basically effect will end like 5 seconds after first scream or something, then it's fine perk imo.

Should the perk go on cooldown early if the killer hits a survivor with a basic or special attack, to limit snowballing?

You can use this against the killers what i advise you do if it triggers is don't run around like crazy panicking leaving scratch marks everywhere but to move away or hide. More often that not when i have used this perk it's the scratch marks that's left behind from panic that makes me find a survivor not ultimate weapon itself. I applied this when i have been survivos an it's funny to watch a killer waste 10-20 seconds looking for you
Also if you're good in chase you can deliberately be near the killer to pull them away from your team mates an the gens run away from any gens you know are being worked on or to stronger areas of the map.

Odd as it may sound my own main use of Ultimate Weapon has been in stealth builds, using the Blindness effect to hide which generator I've Trail of Tormented.
I also think it's unironically a decent choice on Freddy, as it lets you find awake Survivors more easily.

Haha have you ever gone against pig with UW. Trying to remove a ticking trap and you get pulled off and have to start all over again.
