    had this issue once too, it happens when you hit the skill check right at the beginning

    Is this a consistent issue? What is your ping usually?

    I don't want to be the bringer of bad news but this is not a bug. Your picture shows that you hit the skill check barely too early causing failed skill check and Corrective Action that another survivor had made your failed skill check into good skill check. You can see Corrective Action being active next to your perks.
