Same to me, on PC i was using the sole survivor paired with obsession and bonds and iron will, and i had 2 mates down but only one stack of sole survivor.

Same for me, i healed the obsession more than 10 times and still 0 records for the achievements. :((( this patch 4.0 went out to production so fuckted up, filled with bugs. One good reason to stop playing the game.

The same thing for me, iron will not work when i am downed, or if the nurse blinks trough me with that screaming addon. Or if the killer uses that screaming perk, after the scream i can hear the character moaning of pain when injured.
