Found a gen on Shattered Square that's unable to be fixed from one side, discovered it this morning and at first I thought it was probably just me but others couldn't fix it from that side either.

Demogorgon unable to place portals or shred
This happened much later in the game, but out of the blue I found myself unable to "channel the abyss" and shred or place any portals even though I still had one portal available.

Sadako Teleport Bug
This happened in the Coal Tower map, the match was going quite well until I attempted to teleport to a generator that had activated Tinkerer but when I attempted to teleport I found myself stuck mid-animation and unable to move.
