Playing Legion on Father Campbell's Chapel, I was unable to vault this window spot. Didn't lose me the game, but still worth mentioning.
Found this hex totem which turned out to be Lullaby and it was uncleansable. Didn't get a cleanse prompt. None of the survivors were able to cleanse it. Found it in Lery's in the corner of the map. Lery's is such a maze it's hard for me to pinpoint exactly where, so I hope the provided photo is good enough.
