The Hatch exploit
I have had too many matches in which I would've gotten 4 kills but then one survivor disconnects and the other one instantly escapes through the hatch. I understand why the hatch is in the game and it's fine as long as it behaves as intended. However, giving the survivors the ability to open it on demand is just ridiculous. There is zero counterplay to this as a killer.
My suggestion for a fix would be that DCed survivors don't just disappear but stay in the match. They could get crows faster than usual and the killer could kill them when he has time. That way, the killer would still have to invest some time into killing them which is a great change for survivors. At the same time, exploits like this could be avoided.
Another fix would be that the hatch doesn't open (immediately) after a DC. However, that could also be abused by survivors to troll other survivors so I'd prefer my first idea.

Personally, I find it just as much of an ‘exploit’ that killers just leave the second-to-last downed survivor on the ground to keep the hatch out of play. Hatch is there to keep game interesting if Killer is strong, but in my games the killer almost always gets final two down before hooking either, so it rarely serves its purpose.
I’d suggest easiest fix would be for hatch to open once there’s only been one survivor out of dying state for 30 seconds, then the survivor you’re making sit on the ground there for three minutes won’t feel like DCing because you’d be just as likely to hook him when he’s second-to-last as you would in every other case.

Not directly because of the issue above, but a recommended change nevertheless:
The hatch now has a 10 second interaction to be opened by any survivor. A lock visually appears on the hatch until the last survivor is left in the match (unless Exit Gates are open, then hatch remains locked). Once the lock is gone, the hatch can be opened. The hatch gives off a loud sound of screeching metal 5 seconds into the animation, which can be seen by the killer as a loud noise notification.
Problem solved +more gameplay for the last survivor.

I see how that can be pretty frustrating to play against as a survivor. However, there is counterplay. You can either heal the downed survivor or wait for them to bleed out. If you heal them, you might still be able to get all the generators done if one of you leads the killer on a chase while the other one does generators.
However, in the case I mentioned there is no counterplay at all. Two survivors are alive and then suddenly one of them escapes and you don't get to hook any of them without you being able to do anything about it.
Additionally, the situation you mentioned is a result of the killer playing well and/or the survivors playing poorly. In this case, I find it reasonable that the survivors are in a suboptimal position. The situation I managed is also a result of the killer dominating the match. However, here he gets punished for it by having the survivors end the game in the blink of an eye.
So basically, your situation reflects survivors being punished for playing poorly while my situation reflects killers being punished for playing well.
