Death Upon Hooked for the 2nd Time

We were going against a camping wraith, typical, but not the point. I got hooked once, just before phase 2 activated I thought I would try to get off the hook just one attempt, it worked and I got off without entering phase 2. Shortly after I got downed and hooked again in which I instantly died. No phase 2, the game thought I got unhooked during phase 2. What gives?

you probably did get unhooked during phase 2

But I didn't get unhooked by a survivor once the whole game. Why would I lie?

desync issue, it's the same thing when you escape from a trap while the trapper tries to carry you.
