The last gen finished and I hooked someone and blood warden didn’t cover the entire exit gate so two survivors exited through the gap but one surviver just walked straight through blood warden. Like it wasn’t there.
this happened on PS4 version 3.6.2

Was one of the gates already open? Blood Warden won't activate before the gates are opened, even if all the generators are completed, so if that's what happened it wasn't a bug.
