Is anyone else having matches where the survivors will sometimes make no noise when injured?
It happens to me in a lot of matches especially as Spirit. Half of the time it’s like if they’re running iron will, I check post game and only one person had the perk but everyone is getting the effect. No breathing sounds, no footsteps, no grunts of pain, even Dwights jingle bell cosmetic won’t emit noise. (Even with Strider it happens)
I phase to a survivor, I know I’m behind them so I should hear something but there’s no noise. Once I start phasing again sometimes it’ll fix the issue.
as survivor it happens to me as well, I won’t hear any grunts of pain even if I’m not running iron will.

Unfortunately sounds in-game, especially for Survivors when playing as the Killer, are a bit buggy at the moment in some areas. This doesn't happen ALL the time, in-fact it's quite rare if you play a lot, but it still happens from time to time. The issue sticks out the most because this game is very dependent on sounds (especially as the Killer), so you'll naturally notice an issue when you suddenly stop hearing a Survivor mid-chase around a high-walled loop.
Many players have already reported sound issues in-game, and BHVR is well aware of this issue as well, but you can always report it to the Bug Report sub-forum OR find a report on the Bug Report sub-forum and support it. If you're interested, than I can link you to it below.
As for what you can do about it, there isn't really much you CAN do about it... It's just something that happens in-game, but it's not as common as some players make it out to be. Some players get it MORE than others, while other players (like me) rarely have that issue happen at all during a long session. It really depends to be honest.

Had the same thing once, i was playing Steve and i wasn't running Iron Will and he was making no noise at all, even when i was hooked it looked like i was running calm spirit because i didn't scream but i didn't have that on him.

It happens to me every almost every game. But it gets very confusing because one moment the survivor doesn’t emit any type of noise, but the next time I chase them I can hear their moans. It’s a constant on and off and it’s making playing Spirit kind of confusing since she relies on sound.

Sometimes when you're playing a Killer and there's an issue, that issue will "appear more often" as a result. I'm not saying this might be your imagination because you've experienced it so many times before, but I am saying that it might appear that way than it actually is sometimes.
Regardless, there isn't much you can do about it at this time, you'll have to just go through the proper channels I linked above and hope for the best. I do hope your problem is resolved one day.

Some of this stuff doesn't even feel like an accident. I hate when you have someone hooked and know an injured person was really close by. But its as if the moaning of the person on the hook starts to emit from places other than where she actually is. I will be running around confused. It basically masks the injured the survivor and helps them out. All these sound issues do.
Also what is with the sound of the sacrifice when there is just 1 person left? The sound of it is so insanely loud it drowns out the sound of the hatch unless you're right on it and this has costed me more than a few times.

The sacrifice sound is loud in-general, I've never heard of it "drowning out the hatch spawn" when it happens, you can still fainly hear it spawn regardless. The hatch will also not spawn until they're full sacrificed, which means there's a grace period of silence BEFORE the audio for the hatch opening appears.
As for sound issues in-general, that's still a re-occurring problem that still needs to be fixed. There isn't much else I can say and/or do about it, it's just something that needs to be fixed when they get to it.
