survivor spawned at the start of the game injured and with broken status affect

I started a game as the killer on dbd mobile and saw that the icon for one of the survivors said that they were injured and had the broken status affect. i thought that it was just the icon that was bugged and not the player (I was playing the plague), however when i hit that survivor they were downed in one hit. this is the first time this has happened

    This screenshot is from the PC version, and I don't know if it's exactly the same on mobile, but this is most likely what was happening.
