I always lose things

It had been 2 days and every time I escape, i lose add ons (every time) or my item (sometimes). Also if the killer DC's I don't get my points. I was a red rank 1 and the killer DC'd and i lost a pip' which dropped me back to red rank 2 and never fit my blood points.
