Skill check windows not showing up on PS4

I have had this glitch happen to me multiple times now and to a friend once. I can hear the skill checks for repairs/healing etc but can't actually see the window for the duration of the entire game.
The bug is rare as its only happened to me twice but its absolutely gamebreaking
