Is there any 'rule-of-thumb' or unwritten rules stating exactly WHERE one should be watching for the killer when 2 people are on a gen? I just try to watch the other persons back....

Anywhere and everywhere that they could potentially come from. Particularly if the other survivor is a stranger, you can't trust them to warn you if the killer comes near. Most people are looking around constantly when working on a generator to make sure they don't get snuck up on.
