Infinite carrying glitch

Last night I was playing survivor with some friends, we had a full squad. During the match cannibal downed me, and hooked me. After he was stuck in the holding survivor phase for the rest of the match. I’ve tried to recreate it with some friends in custom match today and we’ve been unable to, and none of us was streaming (well the killer was, but he/she doesn’t save the videos)
We were on XB1, I’d accidentally rushed in and out of lockers twice in front of the killer in a injured state, managed to get away via vaulting out the killer shack window, and was quickly downed shortly after. My friends were able to unhook me no problem and I had no real issues the rest of the match.
