PS4 - Key did not disappear

Another survivor had a dull key with a weaved ring addon and milky glass addon. The weaved ring should have made the key disappear from the trial so that the player who had it does not lose the key upon death. However, it remained where she was hooked and killed. I was then able to pick up the key and escape through the hatch. The key remained in my inventory due to the milky glass addon which allows the key to not be lost upon opening the hatch (black lock).
