Xbox - Sole Survivor tokens bug

Survivor played: Nea Karlsson
While using Sole Survivor, I gained a token as expected when the first survivor was sacrificed. However, I didn't gain a token when a second survivor was then sacrificed. It stayed at one token for the duration of the match until I died.
