Well considering the Doctors powers are all about insanity and madness I could see it being interesting with a Mind Flayer or Chtulu looking skin. But that might be bit weird seeing them shoot out lightning and laugh like a madman.

But beyond something like the joker, where the (doctors) laugh would still be weird(and the liscense would be expensive) Im having a hard time imagining specific skins for him.

So I would still consider those the most viable choices for skins.
