Decisive Strike not triggering on unhooks FREQUENTLY

I am a red rank survivor who plays this game often and I keep frequently running into an issue where after I get unhooked and soon after I jump into a locker or I get downed and decisive strike does NOT activate. Basically after I get unhooked decisive strike is not activating. The skill check does not even pop up AND when I look at the perk it does not go on cooldown at all.

    Had this today and once before really annoying against a tunneling Myers and devs won't acknowledge it's happening as it seems to be one off occasions

    I've not seen this happen at all, so if you could get hold of some video of it happening to you that would be a great help.
