I have just begun to play Myers, for about a week.
It appears the tombstone addon does not work when Myers grabs someone out of a locker, from a pallet and I just had a case where I got a hit at the exit gate barrier and it didn't trigger and the survivor got away.
Myers probably needs reworking anyway, but his most popular addon should always work if I manage to grab a survivor and have reached Evil Level 3.

You need to do a quick attack not a lunge or a grab. It can be annoying when you want the achievment and they use locker, but surprisingly survivors dont want to die.
