(PC) Downed from across map, weird teleport, still downed after being healed, but not really(?)

Playing on PC
Was playing normally, but was downed from across the map. While recovering, I got weirdly teleported to inside a rock, and then back to where I was. After being healed, I was still in a downed state, but had prompts to dead hard and vault. I didn't get to try them though.
I have no idea how it could be reproduced.
Here is a full video of what happened, unfortunately I didn't get the teleport part, only the other bits.
This is the only time that this has happened to me.
BHVR, pls fix
