Bug: Unable to grab a downed survivor (PC)

While one survivor was hook anther survivor at injured state (half health) attempted to an unsafe unhook and was downed that survivor then crawled on bottom of the hooked survivor and I was unable to grab him. Later I managed to end up in the same predicament with the other 2 survivors. SWF exploit?
Other info: I was playing as Killer: Nurse, Perks: Iron Grasp, BBQ, Undying, Ruin Map: the western looking one
