Died Immediately on Second Hook?

Hi, Xbox player here. Played a match tonight where I died immediately upon being hooked my second time. Struggle prompt never even appeared, I just went straight to sacrifice animation. With there being a literal hook counter in the ui now, I made a recording to double check and indeed it was only my second hook. I have played a fair amount of survivor this patch and I've never seen anything like this.
