Losing Items on Escape

On PC, if you bring an item and escape, regardless of how much charge you use, you lose it.
A friend of mine had a yellow medkit and lost it, I had a flashlight that I hadn't used any charges on, and it disappeared from my inventory. Both of us escaped. A new item seems to remain in your inventory, however.
This occurs every game.

This happens to me with keys I don't have to use all the time.
I escape through an open Exit Gate or Hatch and the game deletes my key, as if I had to use it.
