Survivor getting stuck on hook/during unhook

Playing on PC, please watch the attached video. I couldn't see it from the distance but it looked like she was hit with some knives while she was getting unhooked, which caused her to get stuck on the hook like that while her icon shows her as unhooked. I could even sabotage the hook she was still on. It stayed like this for the rest of the game.
