Cannot pickup survivor cause they are on a chest

The moment I was taking a screenshot of the issue the Adam crawled off the chest. Though the proof is there that blood is on the top part of the chest. I play on PC from steam. This issue happened today though only once in my match. If you require further information about this I would be glad to clip a small part of my stream of when this issue happened and how.
