Decisive Strike Not Working When Unhooked 2nd Time

I keep encountering this issue where Decisive Strike will not activate when I get unhooked for the second time. It works normally when I get unhooked the first time, but when I'm unhooked the second time, it will sometimes not activate. I'm not sure how to replicate this, because it happens at random.
I am playing on PC
I am playing Dwight Fairfield
The perks I am using are Balanced Landing, Prove Thyself, Borrowed Time, and Decisive Strike
This can happen on any map
This issue will happen every other game at random
