It's very rare for you to get injured and not get into a chase immediately
It's usually from escaping a trap, sniper hatchets, Wraith and Hag's hit-and-run playstyle, or Legion and Plague
Lucky Break is in a pretty nice spot currently though I would like the timer to pause while being healed
