Quality of Life Buff to Coulrophobia
There are perks such as spine chill that light up based on meters, meaning that the mechanic somehow exists and could be applied to other perks ... well, one of the main issues with this perk is that it lacks visual feedback an indicator for when it's working.
So .. give it the same effect, make it so when it's not affecting anyone it goes dark and when it is, it lights up. This way one can better analyze the value that you are getting from it, and it also gives it an additional buff of knowing when survivors are within your terror radius so a bit of tracking like whispers.
I feel this perk has a lot of potential but people don't enjoy perks that they can't confirm are doing it's job, this way you could make a guess on how many times you got value from it. Currently it's only used in niche builds mostly with doctor.

My buff: when the survivors leave terror radius, the effect stays for 10s.

That would make it kinda like an hybrid between Whispers and Nurse's Calling. That's interesting.

That's actually not a bad idea.

That could also work but I wonder if it would need a cooldown in order to not continuously apply the effect. Like the killer leaves, the 12secs expires, then the killer returns and there's another 12 secs - could potentially be eternal lol.

I mean if you think about it, survivors are alerted to its presence so why shouldn't it work the other way around as well?
Or we could just take the alternative route and have Coulrophobia NOT be revealed

Wouldn't it still affect the bar so it's basically revealed? I mean honestly I would be fine if they reduce the healing slowdown by 10% but add 6 meters to the terror radius so it reaches from a longer distance. Right now it's too dependent on either distressing or killers like doctor.
But really the notification of when it's working would be enough of a buff imo to make it more viable since it adds the bonus of a bit of tracking.

Okay there are 2 problems with your idea. One, because of Nurse's Calling, survivors know to never heal in a terror radius. Two, having 2 perks that do the same thing is kinda redundant.
Here's how I would change it. Keep the 50% heal debuff in the terror radius but have the perk light up when a survivor heals OUTSIDE your terror radius. Or even better, keep your perk idea the same and have Nurse's Calling reveal auras of healing survivors 24 meters away from you. I don't know, just don't make it redundant

But then it would just be whispers but a lot better since you could also track survivors with it.

This is a good idea i hope they implement it

Technically you would only get the notification when a survivor starts healing since the notification only appears when you do so. In other words, it's a worse Nurse's Calling with an added benefit

Its still a bad perk, there is no need for this rework.

I mean either way its revealed right? You are going to know its there the minute you start healing someone.
