Step 4 : the incapacitated effect icon on the right counted down the timer, but remained on my screen.
Step 5 : NOT able to work on gens, heal myself or others, use or drop my flashlight. could ONLY vault/throw pallets, and vault windows.

This happened to me on Switch. Huge problem, especially since the killer and other survivors can't see I'm incapacitated and I couldn't signal that I was incapacitated and unable to do anything.
Specifics: I was playing as Dwight on MacMillan. Killer: Nemesis. The incapacitated effect was caused by Eruption.

I was just in a match against the Nemesis, and in his loadout he carried Eruption. I was doing a generator and the the timer displays as intended following my gen to blow up. After the 16 second maxed timer ended, the perk's symbol still appeared on the right side of the screen. I could not continue repairing on that gen. So I look for another generator, and there's no prompt or ability to work on it due to eruption still be active. So since I couldn't repair any gens whatsoever, I decide to go unlock the vaccine box that was highlighted, (since I was infected at the time) only to find out that I could not open it. Now unless he had other perks that could effect this perk to last infinitely once activated, then it may not be a bug. However, that is way too OP for a survivor to not repair ANY generator on the map, and it guarantees a win for the killer that uses the perk.
This video is 4 minutes long, so feel free to skip from start to end. This basically explains more than I can type.
