Getting Sacrificed on 2nd Hook, NOT Death Hook

On PS4. Seeing that my team wasn't going to make it before I went into stage 2, I attempted a Kobe and succeeded. I was near stage 2, but not at stage 2 (or else I couldn't have Kobed). The next time the killer downed me, I was instantly sacrificed to the entity. No, I was not the last survivor alive. A friend of mine told me he had a similar experience recently. He unhooked someone (near stage 2, but not at stage 2 yet) and that person also got sacrificed the next time they were hooked.
